Há basicamente dois tipos de abordagens para o tratamento da má oclusão de Classe II. A biprotrusão é o estado em que o paciente apresenta os dentes anteriores da maxila e mandíbula projetados para vestibular, essa condição deixa o paciente mais exposto a traumas dentários, além de comprometer a estética facial do mesmo. Para solucionar este tipo de má oclusão pode ser indicado exodontias. A escolha dos dentes a serem extraídos para o tratamento, geralmente, incide aos primeiros ou segundos pré-molares, por causa da sua posição na arcada. O presente caso clínico apresenta o tratamento ortodôntico de uma má oclusão de Classe I de Angle com biprotrusão dentária, em paciente do sexo feminino com 23 anos de idade. O tratamento de escolha para o caso, foi extração de quatro pré-molares, seguidas por retração dos dentes anteriores com pouca lingualização, pois foi feito torque vestibular resistente para controlar a posição vestibulolingual das raízes, consequentemente obteve-se melhor acomodação dos dentes em suas bases ósseas. O presente trabalho descreve o tratamento clínico ortodôntico de uma paciente de 12 anos que apresentava um mesiodens na dentadura mista e má oclusão de Classe I com presença de apinhamento dentário, diastema e erupção ectópica de incisivos laterais superiores na região anterior do palato. O resultado do tratamento foi satisfatório, a má oclusão estabelecida inicialmente foi corrigida e a paciente alcançou um ótimo alinhamento dos dentes erupcionados ectopicamente, desta forma a harmonia do sorriso e a estética facial foram recuperados.
